Text Box: POWER OF ATTORNEY
HEALTH CARE PROXY WORKSHOP

For Seniors 60 years of age or older…

The Ontario County Office for the Aging, in conjunction with Legal Assistance of Western New York, will host two Power Of Attorney/Health Care Proxy Workshops.  If you want to decide in advance who will control decisions about your medical treatment and financial affairs should you become sick or incapacitated, one of these workshops may be useful to you.

For your convenience, two separate workshop locations will be available.  These are self-help workshops. The setting will enable those who attend to prepare a Power of Attorney and a Health Care Proxy on the day of the workshop.  These important legal documents will be prepared under the supervision of a licensed practicing attorney.
