Ontario County Office for the Aging is a Human Service Agency located at 3010 County Complex Drive in Canandaigua, New York.
Nestled in the Finger Lakes region, the county houses approximately 110,000 residents. The Office for the Aging serves people 60 years of age and older and their caregivers.
The mission of this office is to provide advocacy, information, assistance, assessment and referral.
Many of our services/programs are directly provided and some are provided through various contracts.
New York Connects, a “Single Point of Entry” for long term care services, is housed at this agency.
A “Single Point of Entry” offers a “one stop shop” for consumers attempting to navigate the many aspects of the long term care system.
Calling one phone number, 585-396-4040 or 315-789-1321, will connect you with a representative who will assist you in maneuvering the long term care system by giving assistance, direction and support. The long term care system consists of Nursing Home, Assisted Living and Community Based Home Care.
