There are several health insurance programs that may benefit you or your family. All programs have certain income and/or resource guidelines.

Below is a brief description of some of the insurance programs availability to you. If you live in Ontario County and want to apply for health insurance, you can enroll in health care programs at one of the listed sites.

Child Health Plus

What is Child Health Plus?

Affordable health care for kids. New York State has a health insurance plan for kids, called Child Health Plus. This plan is available through dozens of providers throughout the state. Enrolling in Child Health Plus is easy.

What benefits can you get for your kids?

  • well-child care
  • physical exams
  • immunizations
  • diagnosis and treatment of illness and injury
  • x-rays and lab tests
  • outpatient surgery
  • emergency care
  • prescription and non-prescription drugs if ordered by a physician
  • inpatient hospital medical or surgical care
  • short-term therapeutic outpatient services (chemotherapy, hemodialysis)
  • limited inpatient and outpatient treatment for alcoholism and substance abuse, and mental health

Who is eligible?

Children under the age of 19 who are not eligible for Medicaid and who have limited or no health insurance may be eligible for Child Health Plus. Even if your family income is high, you can enroll your child in Child Health Plus.

What does it cost?

There are NO CO-PAYMENTS for services or benefits received! Depending on your gross family income, you may have to pay a family contribution to enroll in Child Health Plus. Find your annual income on the chart below to see how much, if anything, you will have to pay.

Family Health Plus

Family Health Plus is a public health insurance program for adults between the ages of 19 and 64 who do not have health insurance — either on their own or through their employers — but have incomes too high to qualify for Medicaid.

Family Health Plus is available to single adults, couples without children, and parents with limited income who are residents of New York State and are United States citizens or fall under one of many immigration categories.

Family Health Plus provides comprehensive coverage, including prevention, primary care, hospitalization, prescriptions and other services. There are no costs to participate in Family Health Plus. Health care is provided through participating managed care plans in your area.

Medicaid is a program for citizens of New York State who can't afford to pay for medical care.

You may be covered by Medicaid if:

  • You have high medical bills.
  • You receive Supplemental Security Income (SSI).
  • You meet certain income, resource, age, or disability requirements.

The Centers for Medicare & Medicaid Services (CMS) administers Medicare, the nation's largest health insurance program, which covers over 50 million Americans. Medicare is a Health Insurance Program for people 65 years of age and older, some disabled people under 65 years of age, and people with End-Stage Renal Disease (permanent kidney failure treated with dialysis or a transplant).
