Medicaid Managed LongTerm Care plans (MLTC) provide long term services and supports to chronically ill and disabled people who are eligible for Medicaid. If you are Medicaid eligible and need home care services you will be required to join a MLTC Plan if you:
Have both Medicaid and Medicare (dual eligible)
Need home care, adult day health care, or other long term care for more than 120 days (four months)
Are age 21 and over
What is a MLTC Plan?
A MLTC is a type of health insurance which coordinates all your long term care services through an approved network of providers. A care manager will assess your situation and develop a Plan of Care to meet your specific needs.
The MLTC services include:
Home Health Aides
Personal Care: Help with bathing, dressing and grocery shopping
The MLTC does not include primary, acute or emergency care. Those services are covered under your Medicare or supplemental health insurance.
How to Choose a Plan
If you don’t pick a Plan one will be assigned to you. Therefore, selecting a MLTC is important and it is suggested that you discuss it with your family, doctor or another person that helps you make health care decisions. You can change plans if you are not satisfied with the one you have chosen.
